High grade squamous intraepithelial lesion (CIN 3) with focal
endocervical gland involvement; cauterized margins are uninvolved
C: Endocervix, curettage:
Fragments of unremarkable endocervical tissue, no dysplasia
identified

GROSS EXAMINATION:
A: Received in formalin, labeled
with the patient's ID, labeled on the
container and requisition as "anterior LEEP" and further on the
requisition as "cervix LEEP--anterior lip" is an unoriented pale gray
cervical tissue fragment (LEEP), measuring 2.2 x 1.4 x 0.4 cm. The
ectocervix has a pale gray to tan brown slightly roughened appearance. No
distinct endocervical margin is grossly identified. The stromal margin is
inked in black. The specimen is serially sectioned and submitted entirely
in sequential order in three cassettes labeled A1-A3.
B: Received in formalin, labeled with the patient's ID, labeled on the
container and requisition as "posterior LEEP" and further on the
requisition as "cervix LEEP--posterior lip" is an unoriented cervical
tissue fragment (LEEP), measuring 2.2 x 1.9 x 0.5 cm. The specimen has a
U-shaped appearance, with a moderate amount of semitranslucent mucinous
material attached to the central U-shaped portion. The visible ectocervix
has a pale gray focally nodular appearance.
Inking code: presumed endocervical margin in blue, stromal margin in
black.
The specimen is radially sectioned and submitted entirely in sequential
order in four cassettes labeled B1-B4.
C: Received in formalin, labeled with the patient's ID, labeled on the
container and requisition as "ECC" and received on a Telfa pad is a 1 x 1
x 0.3 cm aggregate of tan soft tissue admixed with mucus, which is
entirely submitted in cassette C1. G-CMA:sas
