CT Pancreatoiliary pelvis postop (contrast)
[Finding]
diffuse, severe fatty liver가 생로 생겼음.
IHD dil. 보이지 않으며 liver 내에 definite한 focal mass 없음.
upper abdomen에서 prominent한 collateral vessel이 관찰됨.
collateral vessel은 splenic hilum에서 부터 gastrocolic trunk까지 연결되어 있음.
splenic vein의 portal confluence area에 short segmental obstruction이 관찰되며 collateral vessel은 splenic occlusion의
secondary finding으로 생각됨.(arrow)
그러나 splenic vein obstruction site에 definite한 mass lesion은 보이지 않음.
recurrent tumor encasement를 감별하기 위하여 좀 더 f/u이 필요함.
기타 op. bed에는 뚜렷한 recurrent 혹은 residual mass의 evidence 없음.
paraaortic area에 몇개이 작은 reactive LN들 있음.
paraaortic duct 에는 small stent가 들어있음.
복강내 ascites 없음.
[Conclusion]
Segmental obstruction of splenic vein, near portal confluence with collateral formation.
No definite soft tissue mass around splenic vein occlusion site.
